Schedule TEAS exam and submit TEAS score (passing: 64% or higher, two attempts per year, valid for one year) Complete Application Portal: application.gurnick.edu Submit $100 registration fee High school graduate or have earned GED and be at least 18 years of age Valid ID or driver’s license To request credit granting: [email protected] or to campus Request official transcripts to be sent electronically to Provide course descriptions for the courses for which you are requesting credit Submit a 2-3 page essay (see guidelines on point system) Submit 3 letters of recommendation to the nursing program written on formal organizational stationery Submit 1 letter of community service/volunteer work (e.g. volunteering at health fairs, in hospitals or clinics, work with homeless, mentoring or tutoring other students). Must substantiated with a document or letter of verification on formal organizational stationery. Submit current resume Submit either: proof of 1 year of LVN/LPN work experience within the last three years proof of recent graduation from LVN/LPN school within one year of the time of application Submit a copy of your active LVN/LPN license
